WHAT DATA DO I KEEP?
When you attend any of my classes you will be asked to complete a form asking for the following information:
Name, address, date of birth, phone number, emergency contact number, details of any relevant medical conditions and medication being taken.
WHY DO I NEED THIS INFORMATION & HOW WILL IT BE USED?
The information is required in case of a medical emergency and to keep you informed with class information.
If you have agreed on the form, you will be emailed a quarterly newsletter.
If you have agreed on the form, you will receive important class details by email or text, e.g. class cancellations, change of time/venue, etc.
Your address will be used to send out occasional free class passes.
HOW IS YOUR DATA PROTECTED?
The original form you completed will be kept at my house in a locked filing cabinet.
All the information you provided will be stored on my phone (in case it is needed in an emergency during a class). This is protected by both a password and encryption.
HOW LONG WILL YOUR DATA BE KEPT?
Every year I will ask you to update your form to ensure your information is correct. If you are no longer attending classes your data will be deleted and the original paper copy will be shredded. You will no longer receive any emails or texts.
OPTING OUT OF RECEIVING EMAILS
If you agreed to receive texts and/or emails on your form, but change your mind and wish to opt out, you can request this by contacting me at firstname.lastname@example.org or 07786516749
